Is it possible to have an ectopic pregnancy after hysterectomy?

Very rare but yes.

This can occur early after hysterectomy if there is an implanted ovum trapped in the fallopian tube or several months to years after hysterectomy if there is a fistula permitting migration of spermatozoids from the vaginal vault into the peritoneal cavity.

Bottom line, consider checking a HCG on any sexually active-ovulating female who presents with abdominal pain regardless of prior history of hysterectomy and/or tubal ligation.
