What type of hemorrhoids should be considered for excision in the ED?

• external, not internal hemorrhoids
  
• thrombosed (bluish mass covered by epidermis)
  
• super painful

Without excision, thrombosed external hemorrhoids will generally resolve spontaneously over 1 - 3 weeks.
